PageForward is no longer under active development, and hasn't been for some time.
The author has completely rewritten PageForward from scratch and has given it a new name:

Listed below are some some common problems you might experience with PageForward and their solutions.
PageForward is not proxifying images; they're still blocked! What gives?
Make sure you've changed the 'proxify_media' option to 'true' in the user configuration section of the PageForward script.
PageForward GUI doesn't work on my server, even though I've closely followed the directions in readme.txt!
PageForward GUI is still experimental and might not work on older versions of PHP. If you are having problems with PageForward GUI, please contact me, and tell me what version of PHP you're runnning and any other information you think might help reveal the problem.
PageForward won't run at all; it gives me an error that reads 'Fatal error: Call to undefined function: curl_init()'!
This is not a problem with PageForward, it is a problem with your PHP installation: your PHP installation doesn't have the cURL extension enabled. If you're the system administrator, uncomment the cURL extension in your PHP.ini file, otherwise, contact your webhost and see if they can enable this extension for you.
